The purpose of the Home Health Aide is to provide personal care, comfort measures and some restorative activities to the patient under the guidance of a R.N. and/or Physical Therapist.
POSITION REQUIREMENTS
Education: High school graduates preferred. Must have a general knowledge of medical terms and must possess adequate spelling skills and legible handwriting skills.
Certification: Certified as a Home Health Aide by a facility acceptable to the Missouri Department of Health, Bureau of Home Health Licensing.
Experience: One year's experience in a health care institution preferred. Life experiences may qualify individual to take approved Competency Exam-written and skills checklist.
Essential Functions:
Ability to accept and utilize supervision.
Ability to establish and maintain constructive working relationships with individuals and families.
Must have the ability to function independent of close nursing supervision, to rely on their own judgment and skills.
Must be able to deliver direct care hours in patients' homes with minimal time in the office.
Attendance meets established standards with no established pattern of absence or tardiness.
Must be present at work site or working in the field seeing patients as assigned (home visits)
Review and respond to emails in a timely manner.
The employee must frequently lift and/or move up to 5 pounds.
Ability to lift and/or move up to 50 pounds. Must occasionally walk short distances.
Frequent lifting/carrying/pushing/pulling, walking and squatting.
Job Duties/Responsibilities:
May carry out and/or assist the patient with the following procedures: (The Home Health Aide may only perform those tasks in which he/she has demonstrated competence as determined by the R.N. or P.T.)
Reports changes in the patient's condition and needs to primary care R.N. as needed.
Provides only those services as outlined on the Aide Assignment Sheet. Provides all services in a thorough and safe manner.
Identifies those areas that are to be checked off on the specialized procedures checklist and notifies the R.N. to assess her competency prior to performing the task.
Submits clinical notes daily and other documentation in a timely manner via computer.
Prepare Admission Charts and file field charts if requested.
Achieves satisfactory productivity measure of 100 weekday visits per 4 week period and as schedule will allow.
Helps out in other programs as available and completes documentation for those visits as needed.
Participates in patient case conferences or IDG meetings if requested.
Cooperates with all departments to ensure quality service.
All duties are performed in a manner consistent with the VNA standards and values.
Attend InServices and trainings as required by agency department
Other job duties are performed as needed or requested.
