Role and Responsibilities

Train to provide personal care and supportive services to individuals in the community, in the privacy of their home.

Essential Functions

• Train to provide health-related services as outlined in the plan of care, authorized by a physician, and established by a registered nurse or licensed therapist, under the direction of the Aide Matrix established by the NYS Department of Health.

• Train to provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home.

• Train to be able to complete and submit necessary patient documentation including charting tasks and observations on android device.

• All other duties as assigned.
