Traditions Home Health Services is currently seeking a compassionate, skilled, reliable, and experienced Home Health Aide to assist with our geriatric aged couple who resides in the Hyde Park area.Available Shifts:Evenings 5p-11p.Weekdays and Weekends flexibleHHA Job Responsibilities:- Assisting with daily living activities, including toileting, incontinence care, dressing, grooming, nail care, oral hygiene, and skin integrity maintenance.- Monitoring vital signs, monitoring O2 tubing throughout your shift, facilitating daily exercises according to the client's care plan, performing range of motion exercises, and walking- Preparing meals in accordance with the client's care plan, client also participate with Meals on Wheels and ensuring adequate fluid intake throughout the shift.- Adhering to infection control protocols, wiping down counters, cabinets, doorknobs, and tables with antibacterial wipes.- Administering medication reminders and assisting with the application of eye drops, ointments, creams, and ear drops.- Conducting household support such as laundry, folding, putting away clothes, changing sheets, vacuuming, washing dishes, and dusting to maintain a safe and clean home environment for the client.- Engaging with your client in fun, stimulating activities to create joyful moments.- Engaging with your client in meaningful conversations- Skilled in assisting with transfers, operating mechanical lifts, and providing catheter care.- Proficient in dementia care- Competently documenting shift activities digitally using a smartphone throughout your shift- HHA would report any concerns to the Traditions Home Health Services Office Support Team in a timely manner
