For more information about Aspen Ridge Home Health and Hospice visit aspenridgehhh.com For more information about the LHM Group, visit www.lhm.com.Qualifications* Graduate of Medical Social Worker program.* Possesses current registration or license as required by the state of practice.* Minimum of two (2) year experience in a health care facility or equivalent experience preferred.* Home Health experience preferred.* Demonstrates good written and verbal communication skills interpersonal skills.* Licensed driver with automobile insured in accordance with state/Agency requirements. Good driving record and reliable transportation.* Current CPR certification.ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY* Performs Medical Social work assessments as well as ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.* Completes initial assessments in forty-eight (48) hours of referral unless other arrangements are made.* Documents all findings, plans, interventions, and client progress (outcomes) on designated forms and submits per Agency guidelines within (48) hours.* Communicates plans and changes to the Physician and to Case Management. As well as all other Agency staff on changes to the care plan. Completes written progress notes timely and participates in care conferences and collaboration meetings.* Implements appropriate education based on the client's needs and within the context of his/her environment and learning ability.* Plans, teaches, supervises, and counsels clients and families about their community resource needs, in order to promote symptom management of disease process as well as overall improvement and recovery.* Evaluates the effectiveness of the teaching plan and makes adjustments in order to fulfill client's goals and objectives to the highest possible level.* Uses OASIS assessment tools and outcome reports to improve services and client outcomes.* Demonstrates knowledge of safety/infection control practices by compliance with agency policies and procedures.* Assesses safety of environment and takes initiative to help prevent accidents and promote safety.TYPICAL PHYSICAL DEMANDSRequires a full range of body motion including the handling and lifting of patients. Manual and finger dexterity and eye-hand coordination required. Occasionally will lift and carry items weighing up to 50 pounds or greater. Requires corrected vision and hearing to normal range. May require working under stressful conditions or working irregular hours. Involves exposure to communicable diseases.
