Position: Home Health RN Scope: The Home Health RN manages patient care in home settings, conducting assessments, coordinating with physicians, and delivering skilled nursing care. Qualifications: Graduate of an accredited nursing program. Current RN licensure in New York. Prior home care experience preferred. Excellent communication skills. Valid driver's license. Duties: Perform home care assessments. Develop individualized care plans. Implement the Plan of Treatment, including wound care, injections, medication management, etc. Provide skilled nursing care. Supervise home health aides. Communicate with physicians and interdisciplinary team. Document patient care accurately. Participate in quality improvement initiatives. Adhere to infection control protocols
