Your Role as a Home Health Physical Therapist:

• Conduct initial and ongoing comprehensive assessments at appropriate times
• Monitor and assess changes in patient status
• Adapt treatment plans as necessary based on patient progress
• Document all treatments and skilled interventions
• Educate patients, families, and caregivers to support goal achievement

Key Requirements:

• Active Physical Therapist licensure or certification in Arizona
• Minimum of one (1) year of Physical Therapy experience
• Experience in a home care setting is preferred
• Valid Arizona Fingerprint Clearance card
• Current CPR Certification (demonstration course required)
• No Covid Vaccination Required
