TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Tecumseh, Nebraska, 68450 About The Position Job Title: Community Health Nurse RN Job Specialty: Home Health Services Job Duration: Full-time Shift: 8-hour day shift, Monday to Friday, with some possible weekend visits Guaranteed Hours: 40 hours per week Experience: Minimum of 1 year of experience in Home Health preferred, but training available for the right candidate License: Current Registered Nurse license in the state Certifications: Graduate from an accredited school of nursing Must-Have: - Active Driver's License - Auto Insurance active and in good standing Job Description: - Perform duties under the general medical direction of the Director of Home Health. - Participate in planning public health and education programs. - Conduct home visits, design patient treatment plans, and set patient care goals. - Serve as a liaison between patients and physicians. - Attend staff meetings. - Assist with weekend patient care coverage as designated by the Director. - Participate in public health screenings and follow up on clients or refer them to their physician as appropriate. - Provide health education to patients and caregivers. - Evaluate and assess individuals and families, considering physical, social, cultural, emotional, and environmental factors. - Interpret guidelines to determine the patient's payment mode. - Work collaboratively with the departmental Director to update care quality. - Accurately complete all required documentation for patients. - Work with area hospitals, nursing homes, and physicians regarding discharge planning as needed. - Stay current on changes to OASIS and Coding guidelines. - Perform any other duties deemed necessary by the Hospital Administrator.
