Job Duties: Perform or assist clients with personal care activities as prescribed in the plan of care, including but not limited to hygiene, mobility, and elimination. Provide and maintain a safe environment for clients. Use equipment safely and correctly (footstools, side rails, Oxygen, etc.) Practice standard precautions in all contacts with clients. Perform or assist client/family with home maintenance activities as prescribed in the care plan, including, but not limited to, light housekeeping and meal preparation. Monitor adequate fluid and nutrition intake, temperature, pulse, respiration, skin, essential elements of body functioning, and changes in body functioning and record/report appropriately. Practice safe transfer, ambulation, normal range of motion, and positioning techniques. Identify the need for intervention by other home care team members on an ongoing basis. Respect all clients' and families' autonomy and right to confidentiality. Document care provided and response of client/family. Submit documentation promptly. Consistently meet or exceed Good Hands Home Care's established service standards. Use supplies and equipment effectively and efficiently. Complete all required agency services each year. Maintain regular attendance. Adhere to Good Hands Home Care's Code of Conduct. Positively interact with all staff, supporting Good Hands Home Care's mission and vision. Comply with Good Hands Home Care's general orientation and agency policies and procedures, including confidentiality and HIPAA guidelines.Job Requirements: Position requires a high school diploma or equivalent. Applicants must be 18 years old at a minimum and must pass Good Hands Home Care's multiple choice competency exam with a score of 80% or higher. This exam tests confidentiality; consumer control and the independent living philosophy; instrumental activities of daily living; recognizing changes in the consumer that need to be addressed; basic infection control; universal precautions; handling of emergencies; documentation; recognizing and reporting abuse or neglect; and dealing with difficult behaviors. It also addresses bathing, shaving, grooming, and dressing; hair, skin, and mouth care; assistance with ambulation and transferring; meal preparation and feeding; toileting (bathroom assistance, incontinence care), and assistance with self-administered medications. Applicants must pass a background check. A candidate is disqualified for any felony conviction or any misdemeanor conviction related to fraud or the endangerment of a child, elderly, or disabled person. Domestic travel to patients' homes, nursing homes and inpatient units is required
