RN Case Manager with **$10,000 Retention Bonus** Comfort and support for end-of-life care in your home, in a nursing home, or assisted living facility. We are your local, not-for-profit Hospice for wherever you call home. Under the direction of the Patient Care Manager, the RN Case Manager for Home Healthcare, Hospice & Community Services (HCS) provides and directs the provision of home nursing care, based on agency policy and procedures, through the competent use application of the nursing process. This is a full-time, benefited opportunity that works, Monday - Friday, 37.5 hours per week, serving the Peterborough, NH area. Responsibilities include, but are not limited to:Assumes responsibility and accountability for the practice of professional nursing in a manner consistent with the NH State Nurse Practice Act and American Nurses Association Standards for Home Health Nursing.Implements teaching appropriate to the client's needs and within the context of his or her environment and value systemPractices nursing with respect for individual, cultural and spiritual differencesCoordinates delivery of care along health care continuum utilizing community resourcesDemonstrates teamwork and effective communication to accomplish client, team and agency goals Requirements Graduate of a state-licensed basic nursing programPreparation in Community Staff Nursing through a National League for Nursing approved programWith at least one (1) year professional nursing experience under appropriate nursing supervision; orA combination of education and experience that demonstrates the person has acquired the necessary competencies required for the positionPossession of a current State of New Hampshire license to practice as a licensed practical nurseMaintain current cardiopulmonary resuscitation (CPR) certificationMust be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS' policies and proceduresSensitivity in human relationships and able to cope with family emotional stress and tolerant of individual lifestylesSkill in using psychological principles in both one on one and group situationsKnowledge of measures to comfort the dying. Emotional stability, flexibility and respect for various religious and spiritual beliefsValid driver's license and access to an automobile during working hours that meet the State of New Hampshire's registration regulations and auto liability insurance.Experience:Nursing: 2+ yearsLicense:RN License in NH
