Shift: Day 5x8-Hour (08:00 - 16:30)Description:Local Travelers allowed. If Traveler's permanent address is within 50 miles from OhioHealth Grant Hospital, 111 S Grant Ave, Columbus, Ohio 43215, the local rate will apply (*** less).Home Health RNSchedule: Typically 5x8's Days 0800-1630On-Call/Weekends: Every 3rd weekend, on-call once weekly as neededRate is all-inclusivePatient caseload: 25-30 patients for case managers, 6-7 visits daily for non-case managersYears of Experience: Minimum 1 year in home healthLength of assignment: 13 or 26 weeks availableAdditional information: New EMR HomeCare HomeBase implementation occurring, will receive training on new EMR.
