Hospitalist Service

Yale New Haven Hospital

Schedule: Per diem/As needed

Description

Yale New Haven Health, the largest and most comprehensive healthcare system in Connecticut, is inviting Advanced Practice Providers to join an established Hospitalist Service at Yale New Haven Hospital.

This position will provide care to patients at home in our Home Hospital program.

Shifts are 8 hours duration Monday to Friday including one weekend (Saturday and Sunday) every 4 weeks.

Qualifications

Qualifications/Education:

Graduate from an accredited Advance Practice Nurse Practitioner Program or accredited Physician Assistant Program with a total of 4 years of clinical working experience as an APP within the inpatient settings or combination of inpatient and outpatient settings.

Licensure/Certification:

Connecticut State Licensed PA or APRN Practitioner License

Board certified

ACLS/BLS Certified

DEA License

Experience:

Prefer clinical working experience as an APRN within the given both outpatient and inpatient settings, but none required.

Special Skills:

Must be able to travel to patient homes in accordance with Home Hospital home visits. This position does require travel.

Physical Demand:

This position requires travel to patient's homes to provide acute and restorative care services within the home environment.
