Job Type Full-timeDescription We are looking for a Family Health Educator to work in the Family Spirit Program with our Community Health Center in Southeastern Utah. Our clinic is a dynamic place to work, practice, and grow. We have 4 primary care health centers and deliver integrated services including behavioral health, Pharmacy, Dental, specialty referrals, chronic disease management, health education, and much, much more.

Family Spirit Home VisitorSpecific responsibilities may include, but are not limited to:
- Serve as a Family Health Educator for the Family Spirit project in Montezuma Creek
- Recruit young mothers to participate in the Family Spirit Program
- Visit young mothers in their homes and teach a comprehensive parenting curriculum
- Provide social support and connect young mothers to other community resources
- Maintain data forms and tracking logs to document study activities
- Represent the Family Spirit Program and the organization's other programs during community meetings and in-service presentations for community partners and local service providers
- Travel to other sites for trainings, meetings and other duties as needed
- Assist with other organization activities as needed
- Service 15 Families with 85% goal of visits with lessons completed.
- Enter visiting data into Visit tracker home visiting database after every visit.

RequirementsEducation and Experience
High School Diploma or GED required. Bachelor's degree in a health or social services field, preferred.Minimum of one (1) year of work, school or volunteer experience beyond high school required.Ability to speak Navajo preferred.Skills

Current driver's license and good driving record (hired candidate will need to provide a copy of a current driver's license in the state where the position is located)Ability to travel with overnight stays required as needed. - Proof of up-to-date PPD skin testing, if previously negative (OR) Proof of positive PPD skin test with the most recent chest x-ray and treatment history.
- Tetanus, diphtheria, and pertussis (Tdap).
- Varicella titers, history of varicella or varicella immunization series.
- Influenza vaccine for the current year.
- COVID-19 vaccine
- Any other vaccinations as requested.
