Provides patient care in accordance with plan of care established by Hospice Care Registered Nurse and other interdisciplinary group members; performs job functions in a fiscally responsible manner consistent with the agencys mission and vision; utilizes sound time management skills in planning day-to-day activities and fulfilling job duties. Performs emptying and recording of patients drainage collection devices; performs and assists patients with activities of daily living: bathing, grooming, shaving, oral hygiene, with special attention to skin care, perineal care, foot and heel care, turning and positioning, passive and active range of motion exercises, progressive ambulation with or without use of assistive devices, and changing bed linens; lifts patients and exercises care in moving patients, so as not to aggravate injury or disease; participates in fall prevention rounds including documentation as required. Identifies and promptly reports changes in patient focusing on pain and symptoms; also reports changes in patient needs or condition to Hospice registered nurse or Team Manager; documents care provided in accordance with HCN policies and procedures, Joint Commission standards, state and federal regulations. Keeps accurate up-to-date record of patients Activities of Daily Living (ADL) and care provided in accordance with physicians orders and plan established by nurse; completes records in accordance with agency guidelines; ensures that documentation is accurate, complete and legible. Perform a variety of light housekeeping tasks essential to patients health care; keep patients area (bedroom, bathroom) neat and clean by emptying garbage can at end of shift and whenever necessary and cleaning patients bedroom once a week and as necessary; change patients bed sheet daily and as necessary; keep linen cart neat and organized and replenish cart at end of shift; assist with ordering of linens. Check supplies daily; assist with ordering; put stock away when delivered; change sharp containers when filled; keep temperature logs and refrigerator logs complete; provide care / assistance to patients during an emergency. Immediately alerts nurse and the team leaders to the emergency; communicate effectively with patient / family / caregiver and members of the interdisciplinary group. Provide clear, direct, and timely verbal and/or written information regarding patients condition and family concerns; demonstrate appropriate and efficient use of supplies and equipment. Consistently meets the productivity standard established by the agency; participates in on call rotation as appropriate; attends scheduled Hospice Aide In-service meetings and complete annual mandatory hours of in-services. Certification by New York State Department of Health approved home health aide training program, required. Current and valid drivers license. Minimum one (1) year experience in hospice, homecare and or nursing home setting, preferred.
