UPMCis hiring acasual Home Care Aideto support theirHome Health&Hospice areas!Will be required to travel to hospice patient residences within the Family Hospice Wellsboro and Cole territories.

Day light hours - No on call! Rotating weekend and holiday availability may be required to support the hours of operation.

Purpose:Provide designated care under the direction of the primary service provider in the home in order to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.Responsibilities:

  • Must maintain annual competency evaluation scores as determined by regulatory requirements
  • Attends compliance training and adheres to the organization standards of conduct, policies and procedures.
  • Proven ability to facilitate change.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
  • Identifies, develops and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
  • Performs duties and job responsibilities in a fashion which coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians and other departments within the organization.
  • Participates in all required agency meetings.
  • Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patients condition to appropriate professional staff.
  • Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
  • Provides safe and direct personal care to patients in the home setting under direct supervision of appropriate professional.
  • High School Diploma or GED - Required
  • One (1) year experience - Preferred
  • Certified Nurse Aide - Preferred
  • Minimum of 75 hours of Nurse Aide training - Required
    • If no previous nurse aide training, applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.

Licensure, Certifications, and Clearances:

  • Automotive Insurance
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
  • Driver's License
  • Act 33
  • Act 34
  • OAPSA
