Hospice Home Care Aide- Williamsport and Hughesville area - UPMC - University of Pittsburgh Medical Center : Job Details

UPMC Home Health is hiring a Home Care Aide for Williamsport and surrounding areas.

This position is daylight shift Monday through Friday, 8:00 a.m. to 4:30 p.m. The aide must be able to work every fourth weekend and two (2) holidays a year.

Provide designated care under the direction of the primary service provider in the home to achieve identified goals. Function as an extension of skilled nursing services and/or rehabilitation services in the home.Responsibilities:

• Must maintain annual competency evaluation scores as determined by regulatory requirements
• Attends compliance training and adheres to the organization's standards of conduct, policies, and procedures.
• Proven ability to facilitate change.
• Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient satisfaction.
• Identifies, develops, and participates in process improvement opportunities within the home health agency that will enhance the quality of services we provide.
• Performs duties and job responsibilities in a fashion that coincides with the service management philosophy of the organization towards patients, visitors, staff, peers, physicians, and other departments within the organization.
• Participates in all required agency meetings.
• Communicates appropriately with the interdisciplinary home care team in a timely manner, reporting any changes in patient's condition to appropriate professional staff.
• Maintains and completes documentation accurately and promptly while in the patient's home and within the agency and regulatory standards.
• Provides safe and direct personal care to patients in the home setting under the direct supervision of appropriate professionals.

• High School Diploma or GED - Required
• One (1) year experience - Preferred
• Certified Nurse Aide-Preferred
• A minimum of 75 hours of Nurse Aide training is required- If no previous nurse aide training, the applicant may attend UPMC Home Healthcare Nurse Aide Training Course as available upon hire.Licensure, Certifications, and Clearances:
• Automotive Insurance
• Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
• Driver's License
