Stella Maris offers home health services to residents of Baltimore City, Baltimore County, and Harford County.Stella Maris is looking for Home Hospice RN Case Managers to join their growing Home Hospice team. The hospice nurse will ensure the highest level of care is provided to our patients as they transition to end-of- life care. Professionalism and compassion are essential to success in this role.Assess and admit patients to home hospice programs.Develop plan of care in collaboration with patients, families, physicians and members of the care teamProvide new patients with information on home care hospice services and provide initial patient educationMaintain proper documentation in the patient health record to maintain regulatory complianceProvide both physical and emotional support to patients by honoring the patient and family with compassion and care at this pivotal time.#INDHPQualifications:• RN or LPN licensure in the State of Maryland• Current Cardio Pulmonary Resuscitation (CPR) certification• Prior experience in hospice or home health strongly required• Valid driver's license and reliable transportation• Certified Hospice and Palliative Nurse (CHPN) a plus• Commitment to Stella Maris Hospice's philosophy of care, values, mission statement and organizational culture• Willingness to serve on-callMin: USD $38.05/Hr.
