Angel Care Hospice and Palliative is currently seeking experienced Hospice Aides (CHHAs) that are able to provide amazing care for our patients and their families. Our patients reside in their own residences, assisted living facilities or nursing homes throughout Southern California. Our Hospice HHAs are able to maintain a flexible schedule and communicate directly with patients and caregivers to coordinate their visit schedules. Knowledge of Hospice MD software for documentation is preferred. The CHHAs we are looking for would have excellent communication and time management skills; with an emphasis on superior bedside manners. The key component with this position is schedule flexibility in order to accommodate patient visits on a weekly/biweekly frequencies.
Angel Care Hospice & Palliative is teamwork driven! We go above and beyond to assist our families during a very difficult and emotional time. Our entire hospice team works together to ensure that our patients and their loved ones receive the very best care and service possible. Our Hospice CHHAs are able to rely upon the RN Case Managers and office support staff to assist with answering questions and offering guidance.
Responsibilities
Coordinate with assigned patients to schedule visits at the ordered frequencies.
Communicate with the hospice team regarding patient needs.
Report on patient status using a team-centric messaging system.
Develop effective working relations and cooperate with hospice team.
Take the extra initiative and interact with patients and their families to ensure satisfaction.
Document all visits timely and with complete accuracy.
Adhere to professional standards as outlined by protocols, rules and regulations
Skills / Requirements
Current CHHA Certification - Required.
Proven work experience in the healthcare industry
Effective communication skills.
Excellent organizational and time management skills.
Problem solving skills and ability to multi-task.
Compassionate and caring.
CPR Certified.
