Hospice Licensed Practical Nurse (LPN) - Pruitthealth Corporation : Job Details

To assist the PruittHealth Hospice team in providing nursing care to hospice patients. KEY RESPONSIBILITIES: Provides palliative nursing care and ongoing support in the home as ordered on the plan of treatment and nursing plan of care. Functions as a team member demonstrating collaboration with, and responsibility to patient, caregiver, physician, interdisciplinary team. Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Reassesses the patient and notifies the registered nurse (RN) when pain is not manageable on the current medications. Responds to the physical, mental, social and emotional needs of terminally ill patients and their families. Adheres to all agency, payroll, billing, and documentation policies and procedures. Demonstrates knowledge f medications and their correct administration based on age of the patient and patient's clinical position.
