Hospice LPN Extender - Capital City Hospice – an Addus family company : Job Details

The founders of Capital City Hospice took the lessons learned from their many years of experience in corporate America to create a patient-centered hospice that maximizes resources at the bedside while keeping operations local. Our passion is to provide patients and their families with hospice care that is rooted in the communities we serve. When end-of-life care is necessary, patients and their families are in their most vulnerable time. Our Ohio-based management team, through our locally-based clinicians, can respond to the needs of our patients in a timely and appropriate manner because we are local.

We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Columbus OH area. We don't require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them.

Schedule: Monday-Friday 8a-5p with 2 holiday's on call per year

What You'll Do:

• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, Dietitian or Clinical Supervisor
• Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state.
• Prior home health or hospice experience preferred w/ minimum of 1 year medical/surgical experience.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
