United Hospice is a community-based, not-for-profit hospice, that provides care and support to patients and their families in the Counties of Rockland and Orange, New York.We envision a community in which all individuals and their loved ones facing serious illness retain their dignity and hope while receiving the best care and support that hospice has to offer. United Hospice is currently seeking to fill the following per diem position in our Joe Raso Hospice Residence: The salary for this position is $38 to $49/hr. and is commensurate with education and applicable experience and certifications. Title: Hospice Nurse-RN (Per Diem)Location: Joe Raso Hospice Residence, New City-Rockland CountySchedule: Per Diem - 12.5 hour shifts with weekend rotation as requiredJob Duties:
- Provides direct care to residents utilizing accepted standards of nursing practice and consistent with facility policies and procedures.
- Administers medications and treatments according to established policy and procedure and documents all interventions accurately.
- Performs thorough and accurate resident assessments and provides appropriate follow-up and documentation of all findings and interventions.
- Conducts the admission process for all new residents.
- Participates in hospice sponsored in-service training as required.
- Generates the initial plan of care and performs ongoing assessments and care plan updates in collaboration with the Interdisciplinary Team.
- Performs ongoing symptom assessments of resident and family needs and institutes the appropriate interventions.
- Maintains proficiency in working with technical equipment such as feeding pumps, medication pumps, suction machines, etc.
- Maintains proficiency in managing intravenous access devices including intravenous medication administration.
- Documents in the medical records accurately; completes the entries timely to reflect the residents' condition consistent with the plan of care.
- Collaborates and coordinates effectively with clinical and support staff to conduct the daily care of the resident.
- Informs the physician and other staff of changes in residents' needs and outcome of interventions.
- Documents assessments, interventions, resident responses to treatments, and all communications/conferences/IDT meeting, care plans and discharges.
- Accepts other responsibilities as assigned.
Qualifications:
- Registered professional nurse, licensed to practice nursing in New York State
- Possession of a valid and current NYS Registered Nurse's license is required
- Bachelor's degree is strongly preferred; BSN a plus
- Two years recent clinical nursing experience with strong clinical skills including one year recent Med-Surg. experience required
- Complies with accepted nursing professional standards and practices
- Commitment to the Hospice Philosophy of Care
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Able to communicate effectively, while sustaining a caring, kind and professional disposition
- Must have excellent written and verbal communication skills
- Electronic medical record competence is required
- Admission and end-of-life care experience preferred
