Employment Type:Full timeShift:Day ShiftDescription:​ Full time Hospice Admission RNThe Community Hospice has a rewarding opportunity for a full-time RN to complete Hospice Admissions primarily in our nursing home settings throughout the 8 counties we serve at The Community Hospice.HOURS : 8am-4:30pm (can be flexible). Position does require one Saturday per month.Highlights:Advancement: Strong orientation program, generous tuition allowance and career developmentWork/Life: Positions and shifts to accommodate all schedulesNational mileage reimbursement is provided.What you will do:The RN coordinates with the patient's care team in the LTC setting their medical director and our hospice medical directors to confirm eligibility for hospice services.Will go to local nursing homes to complete initial assessment of patient/family needs and begin implementation including coordination and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, certified nursing assistants, and more!Ensures a smooth hand-off to the Hospice Case Manager.Corporate office is 445 New Karner Road-Albany NY with satellite offices in Catskill and Saratoga NY. Admissions are across 8 counties: Columbia, Greene, Albany, Rensselaer, Schenectady, Saratoga, Washington. Your region is considered in patient assignment.We serve 42 different Nursing homes within our capture areasWhat you will need:NYS RN LicensureASN required, BSN preferredAt least 1 year experience as an RN working in LTC setting highly preferredHomecare experience requiredHospice experience preferredReliable vehicle and driver licenseExcellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.Must be able to document key information concisely and accuratelyStrong computer skillsStrong organizational and time management skillsPay Range: $33.10-$45.75Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
