Job DetailsLeading Hospice Provider seeking LMSW or LCSW for home and facility visits. M-F day time.

• Licensure: A current New York State LCSW and NYS Registration (valid NYS Driver's license preferred).
• Experience: Minimum of 3 years in social work within a healthcare setting; hospice or palliative care experience preferred.
• Core Competencies: Strong customer service, effective communication, teamwork, and professionalism.
• Specialized Knowledge: Familiarity with Medicaid, Medicare benefits, and evidence-based assessment tools.
