Our Services for People with Disabilities team is searching for a dedicated, energetic full-time Contract Monitor for our Host Home Services in the Central and Southwest Iowa regions. Office could be out of Ames or Des Moines office; open to the right candidate within the service region.

This service region encompasses the following counties: Polk, Dallas, Madison, Warren, Jasper, Marshall, Marion, Story, Boone, Harrison, Shelby, Audubon, Guthrie, Adair, Cass, Pottawattamie, Mills, Montgomery, Adams, Union, Clarke, Lucas, Wayne, Decatur, Ringgold, Taylor, Page, Fremont My require travel outside of listed counties to cover service needs and meetings.

Duties

The Contract Monitor oversees specified Host Homes in the Central and Southwest Iowa area, and is responsible for the oversight of services and communicating with the Independent Providers. This role is responsible for:

- Acting as the main contact for interdisciplinary team members and for consumers in the sites

- Working directly with consumers to provide support, assistance, and instruction in areas which facilitate personal growth and independence

This position may require long hours including some evenings and weekends to ensure the safety and satisfaction of the individuals served. Will also be part of an on-call rotation.

Qualifications

Minimum Qualifications: HS Diploma/GED and five years of related experience

OR Associates Degree and three years of related experience

OR Bachelor's Degree and one year of related experience

Preferred Qualifications: Bachelor's Degree in a human services related field and one year of related experience. Additionally, years of related experience in a supervisory or leadership role is preferred.

This position requires a valid driver's license, auto insurance, a good driving record and the ability to pass an extensive background check including checks of the Dependent Adult and Child Abuse registries, Sex Offender registry and a criminal history check.
