Daybreak is looking for a Housing Case Manager to assist in our mission of providing Miami Valley homeless and vulnerable youth with services that promote safety, stability, and well-being. This position will play a role in achieving our vision of a promising future for all youth.Benefits and Perks: The annual salary range for this position is $39,000 - $40,000. Daybreak offers hybrid (1 day a week) and flexible work schedules. About Us: Daybreak was established in 1975 in Dayton, Ohio as an emergency shelter for runaway and homeless youth. Today, Daybreak operates the Miami Valley's only 24-hour crisis hotline and emergency youth shelter and has grown to include outreach, prevention, transitional housing, life-skills education, and other follow-up services for runaway, throwaway, and homeless youth.Position Responsibilities: The Case Manager is responsible for providing Daybreak youth with Case management services and for coordinating client's day-to-day activities to help them accomplish their Individual Service Plan (ISP) Goals. Utilize the Stages of Change Model in the provision of services ensuring client transitions are as effective and efficient as possible and that standards set by CARF and funding sources are met or exceeded by performing the following duties.Provides timely documentation of clinical services per mental health documentation standards for the client record and the billing specialist according to agency protocols. The agency protocol is 72 hours (working hours) between service provided and completion of clinical notes.Meets 40% productivity standard established by the agency and outlined in writing in supervision.Evaluate youth readiness for services or placement in Daybreak's continuum or other programs as appropriate, and reports client's progress to the Senior Management Team.Per client's Individual Service Plan, identify strategies or treatment options to provide adequate services and life skills building activities to restore daily functioning and social skills.Collaborates, maintains communication with, and refers to Daybreak providers such as: Therapists, Psychiatrist, case aide, employment specialist, housing program, shelter program, Oz works, David's Place, Lindy's, SECIP, Daytree and Parenting Edu and outside community providers, to ensure clients advance their ISP goals, reporting client's developments to the Senior Management Team.Collaborate and assign activities within the units' staff per clients' needs and ISP, as they transition from Engagement, Shelter, Opportunity House, or Milestones, as appropriate.Engage, mentor, and meet with clients daily/weekly per minimum standard set forth by the agency's programs policies and procedures, consistent with client's needs and promoting trusting relationships, while providing services and case management.Develop client's schedules to help them track activities and appointments, and accompanies youth to court dates, SSI hearings and advocates for the youth in other official appointments, when appropriate.Assists clients to develop an Exit Plan and provide After Care services per client's agreement ensuring housing stability.Responsible for the integrity of the client's chart maintaining clinical and programs documentation of the youth's progress up to date. This includes but is not limited to intake paperwork, Medicaid and benefits status, crisis intervention, periodic packets, case notes, goals plans, Income/Expense form, Case Management forms, Life Skills Assessment, drug screens, transfer, and discharge documentation.Meet and consult regularly with supervisor, to problem solve client's issues and discuss progress. Consults with Clinical Director as appropriate.EDUCATION and/or EXPERIENCEBachelor's degree (B. A.) in social work or related discipline from four-year of college or university, LSW/LPC preferred; Associate's degree required with two (2) years' experience in program development and case management. At least two (2) years' experience working with adolescents.It is the policy of Daybreak to ensure that the facility is in compliance with all license requirements and that all employees are fit for duty. Once a conditional offer of employment has been given, Daybreak requires that all prospective employees sign a background check release form and pass pre-employment testing before a start date may be given.Background CheckDaybreak is required by State of Ohio law to perform a criminal background check, which includes fingerprinting, of eligible applicants. In addition to the fingerprints, Daybreak requires the following additional background checks.Federal Exclusion databases including:National Sex Offender RegistryOffice Inspector General (OIG)Nurse Aide RegistrySAM of Developmental Disabilities (DODD)Motor Vehicle ReportPre-employment TestingThe following pre-employment tests will be conducted at Daybreak's expense. Tests include:A tuberculin skin test or chest x-ray (by a physician of Daybreak's choosing) A urine drug screening (by a testing facility of Daybreak's choosing)Should the pre-employment reviews indicate that the prospective employee does not meet the employment requirements, Daybreak has the option of either not hiring the person or postponing employment until the condition prohibiting employment has been removed.If the background or pre-employment checks contain information that the applicant has committed, been convicted of, or pleaded guilty to any criminal offense, unless the charge was a minor misdemeanor not involving violence or a sexual offense at least 5 years old, or if the applicant refuses to submit fingerprints for a criminal background check, the conditional offer of employment will be withdrawn immediately.Pursuant to SB 38 (child-serving agency), if it is determined that the applicant has committed one of the disqualifying events listed on the attached Ohio Revised Code Disqualifier sheet may prevent employment at Daybreak.
