The new Flexible Support Services initiative is an innovative model of care that includes funding to address many barriers to long term sustainable housing and recovery.The successful candidate will have unwavering compassion, an open mind in terms of pathways to recovery/harm reduction with respect to substance use disorders, expertise with housing navigation and tenancy preservation, and extensive experience assessing and addressing social determinants of health.Working knowledge of area resources/providers/quality assurance platforms/utilization & review methodologies are a plus. Responsibilities:
- Screen, assess, and develop and implement comprehensive trauma informed care plan for assigned clients that reflects identified needs and treatment preferences of the person served
- Collect and track data correlated to client outcomes and provide reports and trend analysis per contract with respective Accountable Care Organization as applicable.
- Provide comprehensive service coordination through collateral contacts as indicated while leveraging pre and post tenancy support services within a Housing First framework. Complete assessments for Coordinated Entry and/or DMH Services Authorization other state or federal resources as needed or assigned.
- Assist with and participate in strategic planning for the Flexible Services Program.
- Build upon existing partnerships with community-based organizations to cultivate more robust support service opportunities as applicable
- Serve as a member of internal/external multi-disciplinary teams and case conferencing forums as assigned.
- Obtain and maintain current certifications in CPR, First Aid, and all other required training. Ensure that all certificates are current and provide required documentation to the Agency.
Qualifications:
- Requires a Bachelor's degree in social work, counseling, psychology, nursing or other closely related field- or 2 years case management or comparable experience with the homeless population.
- Master's degree in the healing arts is strongly preferred.
- Recovery Coach/Certified Peer Specialists with at least 1-year experience with adult homeless population will be considered.
- Must have strong engagement, support planning/implementation and organizational skills.
- Must demonstrate proven ability to leverage internal/external continuums of care and resources to meet identified needs of people served while maximizing their input and participation.
- Knowledge/experience in supporting homeless adults with mental illness and/or substance abuse disorders. Knowledge of Public Entitlements (EAEDC, Social Security, Food stamps, etc.,) and other resources is required.
- Maintain knowledge of and methods to identify and access community resources - especially housing systems (COC/Balance of State/Permanent, Project Based, Transitional Housing).
- Has working knowledge related to housing navigation and tenancy sustaining supports within a Housing First framework.
Schedule: Monday - Friday, 9am- 5pm. Salary: Bachelors $48,000, Masters $50,000 Bilingual candidates are strongly encouraged to apply and are eligible for 6% salary increase upon hiring. Location: Hybrid role that provides flexible support services in the North Shore, Metro North, and Boston Service areas. This is a community-based/outreach position.
