Southeast LA Area Health Education Center (SELAHEC), in conjunction with the LA Office of Public Health STD/HIV/Hepatitis Program (SHHP), seeks applicants for a Continuum of Care/Housing Specialist. Successful candidate will join a team who share a passion for serving people living with HIV and a team committed to fostering equity, diversity and inclusion to be representative of communities served.
The successful candidate will be responsible for engaging in the coordination and regional implementation of the social determinants of health (SDOH)-related services and activities, as prioritized by community needs and gaps, through project sponsor/sub-recipient technical assistance, training supports and programmatic monitoring. The Continuum of Care Specialist will be responsible for the development, implementation and ongoing monitoring of new assistance and related supportive services for low-income people living with HIV and their families. Specifically, this focus on support services will include housing, transportation and food security. The position is responsible for assuring adherence to all related monitoring guidance and alignment with relevant goals outlined in Ryan White HIV/AIDS Program (RWHAP) and Housing Opportunities for Persons with AIDS (HOPWA) guidance and planning documents. Position also includes duties related to training recommendations, policy research, coordination with evaluation and data activities, promoting SHHP programs and support of the Get Loud Louisiana plan.
The overall goal of these efforts is to improve the health outcomes of persons accessing RWHAP and HOPWA services and an increase in client transition to self-managed care.

Qualifications:
Required: minimum 5+ years related professional experience and excellent verbal and written communication skills and Bachelor degree
Preferred: 3+ years related professional experience and Master degree in social work or public health or related field; demonstrated experience coordinating complex health-related programs of regional or statewide scope; experience in direct service provision. Social work experience is a plus.
