This individual would work with homeless individuals to access housing and community resources as a Housing Liaison. Looking for a caring person who enjoys challenging work with a variety of responsibilities.•Hours: Full-Time, Monday-Friday 8:00 am - 4:30 pm; Participate in the on-call rotation. •Training: Must obtain CPI and SSI/SSDI Outreach, Access, and Recovery (SOAR) certification (will train) •Benefits: Full-Time benefits include, Medical, Dental, Vision and Life insurance. Job duties include but not limited to:•Outreach to the unhoused in the community or those who are at risk for losing their housing. •Establish relationships in the community to engage and enroll eligible individuals in the program.•Attend local Continuum of Care meetings related to coordinated entry/case conferencing.•Act as a housing resource for New Horizons and connect homeless individuals with agency, community, and specialized housing resources. •Refer and transport individuals to initial appointments to establish services for employment, behavioral health and/or substance use disorder treatment, physical health, or other mainstream benefits, as needed.•Establish and strengthen landlord connections within the community.•Assist individuals who do not yet have Social Security benefits to complete SOAR applications and assist them throughout the SOAR process.•Practice Housing First harm reduction and other low-barrier service delivery models.•In collaboration with a Qualified Mental Health Professional, provide clinical expertise as it relates to crisis prevention planning.•Closely track activities and clearly document interventions and activities in progress notes within an electronic health record.•Implement and abide by company policy.RequirementsMaster's or bachelor's degree in a human services field, which includes social work, psychology, sociology, or other related field and/or three years of relevant case management experience in housing; OR any four year degree or combination of higher education and qualifying experience; OR four years of qualifying experience OR Associate of Applied Science in Behavioral Health Support from an approved institution. A valid driver's license and a favorable driving record, as driving is an essential function of this position.
