This position will work closely with Housing Navigators in the HFSC, the Joint Office of Homeless Services (JOHS), and 211 to; identify households who are unsheltered, assist families in navigating the Shelter System openings, and provide diversion and placement supports when appropriate. This position will require a flexible 40 hour work week and can include overtime opportunities through on call responsibilities for overnight and swing shift hours during cold and severe weather advisories. This position works as a member of the Multnomah County's Homeless Family System of Care and will be located at the SEI Community + Family Programs (CFP) department. This position reports to the SEI Housing Program Manager. Self Enhancement, Inc. Community + Family Programs is located at, 2205 NE Columbia Boulevard., Portland OR 97211.PRIMARY JOB RESPONSIBILITIES
- Provide Assertive Engagement navigation services utilizing SEI service delivery best practices for families on the Homeless Family System of Care (HFSC) Homeless Housing Queue.
- Identify and verify eligibility of households on the housing queue and shelter waitlist.
- Assess families as needed for HFSC services and/or refer families to be assessed by the SEI Resource Coordinator.
- Assist unsheltered families in Multnomah County in accessing family shelters, diversion, and housing placement.
- Work to support unsheltered families when cold weather and severe weather have been activated by Multnomah County.
- Coordinate and/or provide transportation to shelter, appointments, interviews, events, and other functions.
- Identify, build and maintain effective relationships with Resident Services Coordinators, landlords, property managers, family shelters and an effective and comprehensive network of community resources to promote housing stability and advocacy when needed.
- Advocate and negotiate with government agencies, and other community partners on behalf of clients.
- Complete all required documentation including Intake and eligibility documents, and maintain participant files, in compliance with the Housing Program Policy Manual.
- Administer assistance as needed, in compliance with SEI Finance Department regulations and procedures.
- Demonstrate the SEI Fundamental Beliefs and SEI Standards in all service delivery.
- Develop quality relationships across the Department and throughout the agency to ensure knowledge and awareness of all SEI programming.
- Participate in providing community education and resource programming for community enrichment.
- Performs other related duties as assigned.
Qualifications
- One year case management experience in Social Services or a related field.
- Bachelor's degree in Social Services or in a related field preferred.
- Must share the SEI Fundamental Beliefs about working with children and families.
- Must be able to demonstrate the qualities of; Strategic, Driven, and Self-Aware.
- Experience working with diverse populations.
- Must have effective interpersonal and problem-solving skills with the ability to interact with people of varying social, economic and education levels.
- Must have the ability to successfully motivate and connect with others.
- Must be highly organized and self motivated with effective time management skills.
- Ability to understand and follow complex written and oral instructions, rules and procedures; to work independently and to be a vital and contributing part of a team.
- Excellent writing skills to include proper grammar, case note writing/formatting, proofreading and editing.
- Advanced computer skills to include the entire Microsoft Office Suite.
- Must have a valid Oregon driver's license and be insurable under SEI's auto coverage.
