Housing Navigator - LHH : Job Details

LHH is seeking candidates with 1 year of experience in Case Management, to assist homeless individuals in LA. At this organization, you will play a critical role in guiding clients through complex housing issues and situations by assessing their needs, developing tailored action plans, and coordinating with various service providers.

This role is Contract to Hire (Temp to Perm) and 100% onsite in LA.

Responsibilities include,

Performing case management responsibilities for 20-25 clients per week

Meet with clients to address any housing issues, conduct room checks and address any housing-related issues

De-escalate situations regarding housing issues or living quarters

Coach clients on life skills including laundry, organization, cleanliness, etc.

Documentation of client progress, entering caseloads and client conversations

Attend group meetings with clients

Qualifications include,

Bachelor degree (required)

1 year of experience (or more) working with the homeless population
