Job Purpose:The Housing Outreach Liaison helps individuals find affordable, safe, quality, and accessible housing. Work with their communities to encourage the expansion of housing options Qualifications: Bachelor's degree in social work, psychology, nursing or human services field, OR At least two years of higher education with two years of experience in psychiatric, substance use treatment or developmental disabilities; OR Any four-year degree with two years of experience in psychiatric, substance use treatment, or developmental disabilities; OR Four years of equivalent experience with consumers and their families receiving psychiatric, substance use treatment or developmental disabilities services to move towards their personal, social, and vocational competency in order to live successfully in the community. Core Job Duties:
  • Explain policies and procedures of the Supportive Housing and Permanent Housing Program.
  • Interpret qualifying information from person(s)-served and determine eligibility.
  • Perform technical and administrative duties in support of Housing Programs.
  • Monitor housing properties for compliance with State requirements and federal regulations.
  • Establish schedules and methods for providing affordable housing services, while utilizing policy and procedures.
  • Reduce burden on current direct service staff to resolve housing crisis issues.
  • Improve client outcomes due to an increase in agency skills to serve clients who experience multiple barriers to housing.
  • Improve institutional knowledge around housing resources.
  • Access specialized training and support.
  • Leverage the existing strengths of the agency to implement effective outreach and engagement of clients experiencing housing crisis.
  • Create a powerful network of experts to assist with resolving housing related problems in the community and statewide.
  • Attend local Continuum of Care meetings related to coordinated entry/case conferencing.
  • Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness.
  • Demonstrate a working knowledge of diagnostic criteria sets for substance use and/or mental disorders.
  • Orally communicate information effectively and accurately.
  • Assure program operates within the constraints of the agency's certification (CARF, DMH) standards and strategic plan related to clinical care.
  • Abide by program and agency policies and procedures.
  • Other job duties and special projects as assigned.
