CRi is seeking a dedicated Housing Specialist to provide essential support to those living in Permanent Supportive Housing (PSH) units. - Direct Support: Work hands-on with individuals in PSH units, helping them develop independent living skills and maintain their apartments.
- Service Planning: Support individuals in locating suitable housing accommodations. Complete assessments and create personalized Housing Plans tailored to each individual's strengths and needs.
- Skills Development: Facilitate training programs focused on life skills, social determinants of health, and community engagement. Support individuals with obtaining legal documentation, income and budgeting needs,
- Crisis Intervention: Identify behavioral changes early and provide timely support to prevent crises.
- Collaborative Relationships: Build strong partnerships with PSH teams, landlords, and community agencies to ensure successful housing stability.
Key Responsibilities:
- Implements the Housing Plan with the individual through housing research, application process, and lease up and move in. They provide on-site apartment support, community engagement and support.
- Develop and implement individualized service plans.
- Maintain effective documentation in compliance with regulations and CRi policies.
- Teach essential skills for housing maintenance and independent living.
- Assess and adapt to the needs of individuals, fostering supportive relationships.
Schedule & Location:
- Monday - Friday 10am-6pm (flexible hours; may require some weekend hours as needed)
- Locations: Fairfax and Prince William Counties
RequirementsMinimum Qualifications Required: Must have a valid driver's license with a satisfactory driving record. Bachelor's degree in a human services field; preferably social work or counseling field. Must have a minimum of one (1) year of experience with assessments, service plan development andservice delivery. Minimum of one (1) year experience providing services to individuals with SMI and I/DD diagnosis. Must be knowledgeable of the following topics:o Treatment modalities and intervention techniques for population identified, to include:crisisintervention or support; independent living skills training, supportive counseling, group work, casemanagement and service coordinationo Clinical assessments and their use in service planningo Individual's rightso Local community resources and service delivery systemso Effective oral, written and interpersonal communication principles and techniqueso Social determinants of healtho Record keeping within EHRs, all documentationo The service planning process and major components of a service plan.Physical Demands:Should be in good physical condition for required lifting and physical intervention for hazardous behavior and assistance with care.Working Environment:Must use universal precautions.Supervision Exercised:None.#ajsj
