Position Overview:
Responsible for providing telephone and in-person screening with families and individuals who are seeking assistance through the Native American Rehabilitation Association's (NARA) Multnomah County resources. Provide regular case management for participants.
Responsibilities:
* Verify eligibility and assist eligible clients in gaining access to rent assistance through an in-depth assessment for service.
* Provide case management to clients at risk of becoming homeless and to help stabilize their housing situation. The goals are income self sufficiency, housing stability and academic success of children.
* Promote family budgeting and self-sufficiency with all clients in case plans and in one-on-one meetings.
* Follow Assertive Engagement model of providing services, including but is not limited to, developing self sufficiency plan for each client, addressing, and assisting client to recognize their strengths, needs, deficits, and goals to access needed resources, and monthly in person contact as well as weekly check-ins with the participants in accordance with requirements of funding sources.
* Maintain current case notes and goal plan for each household and make regular entries regarding the status of their situation and their activities to achieve goals, in accordance with requirements of funding sources.
* Maintain accurate, complete, up-to-date documentation of service activities using NARA procedures, forms, and data reporting systems. Submit paperwork in a timely manner and maintain regular communication with supervisor.
Qualifications
Qualifications:
* Bachelor's Degree in social service/social sciences field with at least six (6) months experience providing housing assistance services or related services; or any combination of education and work experience in social services to total four (4) years.
* Possess valid driver's license, good driving record, own transportation, current auto insurance, mileage will be reimbursed.
* Successful experience with Microsoft Word, Excel and Outlook is required.
* Position will require flexibility in working schedule, may include weekends.
