Key Responsibilities:
  • Coordinate and facilitate housing searches, placements, and success coaching for clients
  • Assess clients' housing needs, strengths, and resources to develop effective support strategies
  • Advocate for expanded housing resources and collaborate with community partners
  • Provide experiential training in self-advocacy skills and resource acquisition
  • Participate in clinical team meetings to contribute to client recovery planning
  • Monitor external services to ensure coordinated care
  • Maintain knowledge of community recovery resources and support systems
  • Ensure protection of client rights and dignity
  • Complete necessary documentation and reporting requirements
Education, Experience, and/or Credential Qualifications:
  • Bachelor's Degree from an accredited college or university in social work, psychology, nursing or a closely related field
  • Equivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED
Additional Qualifications:
  • Must complete background check including criminal record, driving record, abuse/neglect and fingerprint check
  • Current driver's license, acceptable driving record and current auto insurance required
  • Strong communication and interpersonal skills
  • Ability to build effective relationships with clients and community partners
  • Problem-solving skills and ability to work independently
  • Must complete all required training at the beginning of employment and annually thereafter
Physical Requirements:
  • Sedentary work: Exerting up to 10 pounds of force occasionally and/or negligible force frequently
  • Position involves sitting most of the time with occasional walking or standing
  • Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift
