Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000individuals, children and families in three main areas:
* Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
* Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.

Housing Stability Navigator
Full-Time, Monday-Friday 8AM-5PM
Areas of Responsibility: Responsible for locating safe & affordable housing for individuals and families. This position provides services based on the Housing First Model of Care, working with the targeted population to locate & maintain housing stability. Advocates for clients and coordinates CAPWN services and other area resources while assessing clients for eligibility of services for all programs.
Major Duties:
* Knowledge of HMIS database system, Clarity, conduct client interviews completing appropriate intake and required paperwork for entry into agency data tracking system.
* Assess and assist households in locating safe & affordable housing and track client progress in the agency database.
* Work in conjunction with Supportive Housing Program and Family Stabilization Committee in determining client eligibility.
* Collaborate with Northwest Community Action.
* Maintain confidentiality & accuracy of client information and provide a supportive and trauma informed environment for all client interactions.
* Ability to demonstrate active listening skills, conflict resolution, internal motivation, and flexibility.
* Be familiar with current housing market information, housing trends, available affordable housing resources and develop positive relationships with landlords and property management companies.
* Work with clients directly by visiting prospective units as part of a housing search, completing housing applications, speaking, and engaging with landlords and property managers, possible transportation to housing related appointments as necessary, interpreting leases, and understanding tenant rights and responsibilities.
* Assist clients with disabilities in locating appropriate affordable housing that satisfy their needs.
* Complete required housing inspections for prospective units as necessary.
* Assist families with low incomes to apply for federal housing programs like Section 8 or Public Housing and collaborate with other service providers in locating financial resources that may be accessible to them through federal government aid programs, community organizations and other available options.
* Keep clients informed of the status of their applications,
* Network & collaborate with area housing resources, mediate with landlords, troubleshoot housing-related issues with their tenants and provide assistance as necessary to maintain housing tenancy.
* Provide statistical data entry, complete routine paperwork & possible reporting when necessary.
* Cross trains within the other Housing programs as needed.
* Attend and participate in staff meetings and training as requested.

Working Conditions:
* Includes office setting, off-site home visits, training, meetings and community interactions.
* Will include travel in the panhandle.
* Work requires the ability to meet tight timelines.
* Work requires the ability to take initiative and be self-directed.
* Includes out-of-town travel for training.
Physical Requirements:
* Must be able to move about quickly if necessary.
* Physically able to bend, stoop to complete a variety of tasks.
* Occasional lifting and able to carry up to 50 lbs.
* Physically able to perform job.
Minimum Qualifications:
Grade: 11
* High School education or equivalent.
* Two years of documented work experience in a related field preferred.
* Experience with shelter programs homeless shelters, substance abuse programs, social services programs preferred.
* Knowledge of Housing First approach to end homelessness preferred.
* Knowledge or understanding of fair housing, tenant/landlord rights and responsibilities preferred.
* Ability to communicate effectively both orally and in writing and good organizational skills.
* Able to attend meetings and training outside of the immediate community.
* Must be able to pass all onboarding requirements which include but are not limited to Adult/Child Protective Services, criminal and sex offender background checks.
* Must have a valid driver's license with good driving record and proof of insured vehicle.
* Age 21 or above
Grade: 12
* Must meet minimum requirements above AND;
* AA Degree in a related field with at least 1 years' experience OR;
* A minimum of 3 years' experience in a similar position.
Grade: 13
* Must meet minimum requirements above AND;
* BA degree in a related field OR;
* A minimum of 5 years' experience in a similar position.
