Proof of license(s) certifications, and/or education must be submitted with each application.Examples of DutiesThe Human Services Associate will provide case management services to individuals who are incarcerated at the Baltimore County Detention Center and will assist individuals to access treatment and services as they transition to the community upon release. This individual serves as a care coordinator and client case manager. May perform duties that include outreach, referral for substance use, mental health counseling, linkage to community support services to include housing and employment services, and naloxone training. The case manager will maintain appropriate documentation and will provide data for reporting as requested.Examples of Essential Duties* Provides case management.* Assesses, evaluates and monitors individual cases and coordinates care.* Serves as part of a multi-disciplinary team developing care plans for clients.* Advocates the needs of clients.* Makes home visits, transports clients and attends appointments, as appropriate.* Assists in the development and implementation of program policies, procedures and evaluation tools.* Maintains case records, statistical data, and accurate records of client activities.* Prepares reports, documents and correspondences.* Attends meetings and training sessions.Examples of Other Duties* Performs other related duties as assigned(NOTE: The duties and responsibilities listed above are for the purpose of determining a common set of minimum qualifications for all positions in this class. Each position may not be required to perform all of the essential job functions listed.)QualificationsRequired QualificationsGraduation from an accredited college or university with a bachelor's degree in a social or behavioral science.(Experience as described in essential duties may be substituted on a year-for-year basis up to a maximum of four years for the required education.)Licenses and CertificatesSome positions within this classification may require possession of a valid driver's license equivalent to a non-commercial, class C Maryland driver's license at the time of appointment.Proof ofLicenses, Certifications and EducationApplicants are required to submit proof of Licenses, Certificates, and education beyond high school to meet the required and preferred qualifications of the position. Employees are subject to call to duty in emergencies and may be required to work overtime as needed. Employees must satisfactorily complete County sponsored CPR and First Aid training and maintain certification throughout employment.Physical and Environmental ConditionsThese positions are essentially sedentary with occasional walking, lifting, or other limited physical activities.
