Job Summary: Under the general supervision of the Intervention Services Supervisor, the crisis case manager is responsible for the direct provision of services to the general public for the primary purpose of preventing disabilities, promoting mental and physical health, and improving social and community functioning. This includes on-call crisis coverage as it relates to Chapters 51, 54, and 55 providing telephone and mobile crisis intervention, crisis counseling, assessment, and referrals through a rotation of daytime, evening, and weekend hours. The crisis case manager also provides short term service coordination and monitoring of services to a specific group of individuals diagnosed with a developmental disability, emotional disturbance, substance use disorder, and/or chronic mental illness. The crisis case manager is expected to work on a highly independent basis and to work cooperatively with community providers, law enforcement, school personnel, clients, and families.Essential Duties & Responsibilities:
  • Provides direct face-to-face (via walk-in clinic or mobile response) and telephone contact crisis intervention services. Services include conducting risk assessments, providing crisis counseling, supporting behavioral de-escalation through the use of skills coaching, and offering information and referral to a wide variety of collateral agencies, families and resources.
  • Responsible for arranging admission to an inpatient psychiatric facility for individuals subject to Chapter 51 detentions.
  • Works a rotating daytime, evening, and weekend on call schedule with other crisis case managers, with the ability to respond to the local hospital emergency department, office, or local law enforcement agencies within a thirty (30)-minute timeframe. Average 100-120 on call shifts per year. Schedule may vary depending on team members' availability and other job duties.
  • Responsible for ensuring services provided adhere to least restrictive measures and clinically appropriate standards to ensure treatment/recovery is in accordance with best practice and professional ethics and boundaries, and code conduct.
  • Develops crisis plans to address follow up to initial crisis response for continuity of care.
  • Provides liaison services between consumers and community agencies for linkage and follow up to appropriate services.
  • Evaluates the need for protective services, guardianships, and alternative placements for consumers.
  • Collaborates with other providers and agencies within the emergency services system (law enforcement, hospitals, jail, etc.)
  • Monitors and act as case manager to assigned persons with Mental Health Commitments or Settlement Agreements.
  • Generates and maintain clinical records and reports as per state law, program standards and agency requirements.
  • Attends individual and group supervision to staff crisis contacts that are being monitored with the Intervention Services Supervisor, and other relevant program and agency staff during agency staffings. This may include attendance of CSP, Outpatient, CCS, and other department staffings.
  • Occasionally works with inpatient facilities to coordinate services and provide discharge planning services.
  • Processes mental health and AODA court services including, emergency detentions, 51 commitments, protective services, three party petitions. This may include paperwork and attendance at court. Consults with program director regarding cases as needed.
  • Performs other related duties as required or assigned.
Requirements:
  • Bachelor's Degree in Social Work, Psychology, Rehabilitation, Special Education or related field.
  • A 30-minute mobile response time is required while performing on call duties.
  • One to two years related experience and/or training; or equivalent combination of education and experience.
  • Requires a valid Wisconsin driver's license and adequate insurance as required by County Policy.
