Reporting to the Division Manager - Adult/Long Term, the Human Services Supervisor - CLTS directs and coordinates the operations of the Children's Long Term Support Program. Performs related duties including evaluating staff and program performance, serves as a case manager for assigned clients, and performs related duties toward the program objectives and goals of the division.To succeed in this role, it is essential to foster a culture of teamwork and collaborate effectively with the other supervisor in the area.Hours: Monday - Friday 8-4:30 are the typical hours. There may be times when employees have home visits with families outside of the typical hours. This position has comp time available.Remote Work: After 6 months of employment, employees can apply to work 1 day a pay period from home.The following duties are most critical for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.* Supervises intake process; may assign referrals to appropriate division staff for assessment of needs and to provide services.* Obtains information from clients, family members, and other agencies to assess needs, establishes goals, develops and coordinates individual case plans; and monitors established service plans.* Provides individuals and family members with information regarding services which best meet their needs and desires.* Defines, interprets, and provides direction regarding administrative policies and mandates.* Supervises and directs the activities of staff.* Reviews and approves correspondence.* Ensures compliance with State standards and County and Department policies.* Conduct division and staff meetings to discuss policies and services.* Establishes performance standards for division staff; performs periodic staff reviews and evaluations.* Completes program performance reports for State Agencies and Department Management.* Contacts community resources regarding referrals, system problems, and other issues; coordinates division services with other divisions and management.* Represents the program on key standing meetings regarding clients potentially enrolled or enrolled in the program. Represents program guidelines and requirements when discussing client involvement in program and access to County services.* Prepares statistical and other reports as required, and participates in the development of the yearly budget for Outagamie County Health and Human Services.* Perform public presentations to educate and inform the public of Outagamie County's services.* Participates in the development and implementation of agency policies, procedures, and programs.* Maintains working relationships with other human services, medical, and social services agencies in the County and State.* Assists individuals and family members in understanding disabilities and the results of diagnostic and evaluation services. Helps problem solve complaints and inquiries of the program.* In conjunction with Division Manager, interviews and hires for the Program.* Maintains regular and predictable attendance, works extra hours as required.* Performs other duties as assigned.* Bachelor's degree in behavioral science or a related field required. Three to four years of experience with the respective target group preferred.* Or any combination of education and experience that provides equivalent knowledge, skills, and abilities.* Certification as a Long Term Care Functional Screener is required. Training and access will be supplied.* Knowledge of and demonstrated ability to conform to goals, policies, and procedures of the Department as well as the Outagamie County Work Rules and Terms of Employment.* Ability to operate a variety of office equipment including personal computer, dictaphone, photocopier, calculator, and telephone.* Ability to assign, supervise, and review the work of others.* Ability to make recommendations regarding the selection, discipline, and discharge of employees.* Ability to add, subtract, multiply, divide, calculate decimals and percents, and make use of the principles of descriptive statistics.* Ability to prepare a variety of documents including client records, referrals, court reports, intake and assessment records, case notes, individual treatment plans, letters and other correspondence, etc.* Ability to comprehend and interpret a variety of documents including service requests, case files, letters and other correspondence, policy and procedure manuals, union contracts, etc.* Ability to prepare a variety of documents including letters and other correspondence, various statistical and other reports, etc.* Ability to use and interpret social work, counseling, and basic accounting terminology.* Ability to communicate effectively with clients and family members, community agencies and service providers, psychiatrists, physicians, and other medical professionals, attorneys, Human Services personnel, school personnel, and others verbally and in writing.* Supervisory experience preferred.OUTAGAMIE COUNTY IS AN EQUAL OPPORTUNITY EMPLOYER.
