Hybrid Registered Nurse (RN) - 4 Corner Resources : Job Details

4 Corner Resources is actively seeking a dedicated Registered Nurse to conduct comprehensive assessments in patients' homes across the Bronx, NY area. This would be a HYBRID position. Flexible schedule, up to 2 days remote 3 days in the field. Job Details:

• Multiple positions available.
• Duration: 3-6 months; potential for conversion to permanent employment.
• Schedule: Five 8-hour shifts per week, totaling a 40-hour work week. Daytime hours with no weekends required.
• Conduct two visits daily, each lasting approximately 2 hours.
• Organization provides laptop and necessary equipment.

Duties and Responsibilities:

• Conduct home assessments using the Uniform Assessment System (UAS) for both prospective and current members.
• Utilize clinical expertise to determine enrollment and ongoing eligibility.
• Conduct assessments in various settings, including skilled nursing facilities, as necessary.
• Provide feedback to home care agencies and Collaborative Management Team (CMT) regarding care issues and aide performance.
• Review previous assessments and medical notes to update Care Plans or assist with nursing home admissions.
• Deliver community-based training on frail population care, including self-care techniques and prevention strategies.
• Ensure clear understanding and agreement from members and their representatives regarding enrollment in managed long-term care plans.

Qualification Requirements:

• Valid New York State RN license.
• Possession of a valid NY ID or NYDL.
• Ability to travel within the Bronx area.

Pay:

• $52-$56/HR based off of experience.

Schedule:

• Monday- Friday daytime hours
