Hybrid Supervisory Nurse - Caresphere : Job Details

Must Be Registered RN in the state of Pennsylvania or can obtain when hired.

Position Overview:Caresphere is seeking a dedicated and experienced Supervisory Nurse to join our team. This role will primarily involve conducting essential assessments and supervisory duties through Telehealth, with an emphasis on providing high-quality care and efficient case management. Key Responsibilities:

• Conduct Initial Assessments, 60-day Reassessments, and Supervisory Visits for patients.
• Oversee initial and discharge 485 MD Orders.
• Conduct Home Health Aide (HHA) Skill Outs as needed.
• Perform various case management tasks as assigned by the Director of Home Health.
• Regularly review and ensure all assessments are current according to agency guidelines.
• Independently coordinate schedules to ensure timely completion of all tasks and assessments.
• Maintain open communication with patients and their families, providing support as needed.

Work Hours:

• Full-time, 40 hours per week
• Monday through Friday, 8:00 AM - 4:00 PM
• Flexibility to manage schedule effectively to meet deadlines

Requirements:

• Current Pennsylvania Nurse State License
• Successful completion of a State background check and Child Abuse Clearance
• Strong organizational skills and the ability to manage time effectively
• A passion for healthcare and patient advocacy
