Job Type Full-time Description UCFS is currently seeking a full time Case Worker for our Intensive Family Preservation program in our Community Behavioral Health Services department to provide intensive, home-based intervention services to DCF referred families for purpose of preventing child's placement, improve parenting, safety, clinical, and case management interventions along with parent-child relationships. Travel 20%-40% due to location of clients or frequency of face-to-face hours with clients and required full case loads numbers. Essential Responsibilities -
- Process initial referral with clients and referring agents, complete intake and assessment per specific program protocols and time frames.
- Provide program specific weekly face-to-face contact hours with clients to develop a service plan and goals consistent with the purpose of the program and reason for referral.
- Provide therapeutic individual & family oriented interventions related to client's goals and level of functioning per evidenced based model and/or program specifications. Includes responding to own clients' crisis during work-day and rotating after-hours on-call crisis coverage with staff team.
- Facilitate family meetings with DCF, providers and other members of family's team.
- Participate in DCF case conferences, provider meetings, ACRs, and court proceedings.
- Maintain documentation relevant to funder, agency & program requirements. Write termination summaries and reports for DCF as required.
- Provide non-traditional hours, including evenings and/or weekends based on the family's needs. Some limited transport of clients may be necessary.
- Meet required case load expectation, 5-6 families, per program contract.
- Attend weekly team meetings, individual supervision, other meetings and trainings as required by agency & funder.

Requirements
- BSW/BA/BS with minimum of 3 years' experience providing homebased services to DCF families
- Travel 20%-40% required
- Valid CT driver's license required
- CPR certification required within 4-weeks of hire
