ResponsibilitiesSchedule: PRN, Variable Shift, possible weekend assistanceProvides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policyServes as a resource to other healthcare professionals including pharmacy students to ensure that patient care meets standards of safety and effectivenessOversees the duties of the pharmacy technicians and internsEnsures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicableDocuments medication related plans and interventions in electronic medical recordProvides clinical consultation delivering best-practice, evidence-based and complete information to practitionersMakes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaborationOversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patientsThis includes supervision of pharmacy technicians and/or interns who support the role of the pharmacist in the drug delivery systemPrecept pharmacy students, as applicableWorks in a constant state of alertness and safe mannerPerforms other duties as assignedEDUCATION•Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from accredited school of pharmacyREQUIRED PROFESSIONAL LICENSE AND/OR CERTIFICATIONSState of Work Location: Illinois•Pharmacist, Registered - Illinois Department of Financial and Professional Regulation (IDFPR)EXPERIENCE•No experience required, but prefer previous hospital experience
