Job DescriptionFlexStaff is seeking an Incident Management Investigator to join our client's team, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities.This is a full-time, permanent, direct hire position based in Manhattan.Hours are 9-5pm (37.5 work week) this is a non-exempt positionIn this role, you will advocate for the rights of all people receiving services throughout the Agency by conducting investigations into situations, which may violate a person's rights and/or protections in accordance with OPWDD and Justice Center regulations.RESPONSIBILITES
  • Conduct investigations into situations that may violate the rights and/or protection of people receiving services. These situations include all categories and classifications as defined by the Justice Center PPSNA (Protection of People with Special Needs Act) and OPWDD Part 624 Regulations: Reportable Abuse/Neglect, Reportable Significant, Serious Notable Occurrence, and/or 624/625 situations.
  • Complete investigations, IRC Committee recommendations, and follow-up within mandated timeframes.
  • Assist with internal reviews, as needed, with regard to incident management.
  • Ensure investigations are timely, thorough, and address investigatory issues, as well as identify best practices in accordance with agency policies and regulatory standards.
  • Identify areas of concern timely during the course of an investigation needing additional follow-up and/or additional reporting.
  • Participate in regular supervision with the Administrator of Investigations and provides investigative updates, including written notification and follow-up reports, as needed for administrative action. Prepares investigative reports and forwards completed reports to the Administrator of Investigations within required timeframes.
  • Interface with the OPWDD Incident Reporting Management Application (IRMA) for the purposes of reviewing investigative information related to assigned cases. In consultation with Administrator of Investigations, Administrator of Incident Management and/or Senior Vice President of Quality Assurance and Quality Improvement contacts outside entities as needed.
REQUIREMENTS
  • BA/BS in related Human Services Field or licensure in medically related field required.
  • Experience working within OPWDD programs required.
  • (3) years of experience in conducting investigations involving OPWDD programs.
  • Completion of approved Justice Center investigative training course
  • Thorough knowledge of state regulations governing incident reporting (14 NYCCR Part 624 and 625, Article 20 of the Executive Law and Article 11 of the Social Services Law), Protection of Individual in Programs certified by OPWDD (Part 633 regulations), and the Justice Center PPSNA (Protection of People with Special Needs Act).
  • Must have flexible hours and ability to work remotely and on site as needed.
  • Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies, and procedures. Adheres to the agency's corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually.
