Our team of liaisons are the bridge between hospital to home for chronic and acute patients and is the perfect spot for nurses that enjoy teaching, having flexible days and thriving in a team environment.Key responsibilities:* Collaborate with hospitals in the greater St. Louis and Metro East areas including case managers, physicians, referral sources and home health agencies to facilitate coordination of care for home infusion patients* Conduct bed side education and training of IV therapy for patient/caregiver and perform initial patient assessment* Work with pharmacy to determine method of administration based on physician orders and referring hospitals to qualify patients on long term nutrition therapies* Participate in ongoing conversations aimed at enhancing patient and referral source experience* Provide top tier partnership and customer service in conjunction with IV Care pharmacy leadership and Account ManagerEssential Functions: Makes initial visits, educates patients, caregivers and functions as a liaison to hospitals, case managers, nurses and physicians. Regularly re-evaluates needs of the patients. Counsels with the patient and family/significant others on nursing, teaching, and related needs. Inserts intravenous cannulas; teaches and or administers prescribed intravenous solutions, medications, and biological products; monitors and maintains infusion sites and systems; evaluates response to prescribed therapy. Informs the physician and personnel of changes in the condition and needs of the patient. Initiates appropriate preventative and rehabilitative nursing procedures. Provides those services requiring substantial specialized nursing skills. Prepares clinical records, care plans, progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner per company policy. Provides case management along with the pharmacist to enhance patient outcomes. Identifies nursing diagnosis/patient care problems and interventions making additions/changes to ensure the patient's problems are comprehensively addressed to avoid rehospitalization.Competency, Skills, and Abilities: Administer infusion nursing care; instruct, motivate, and assists patients and caregivers in managing their health status. Strong skills in managing Ports, and PICCs. Excellent verbal and written communication skills Ability to prioritize tasks and manage multiple projects. Strong analytical and problem-solving skills Strong interpersonal skillsKnowledge of the regulatory requirements at the state, federal and local levelSolid ability to learn new technologies and possess the technical aptitude required to understand flow of data through systems as well as system interaction.Ability to adapt and be flexible in a rapidly changing environment, be patient, accountable, proactive, take initiative and work effectively on a team.Computer skills including knowledge of Microsoft Office applications.RequirementsEducation and Experience Requirements: Graduate of an accredited School of Nursing. Two (2) years nursing experience in a hospital or similar setting required. Two (2) years of infusion therapy experience. Preferred Must have a valid and unrestricted driver's license. Must have a valid and unrestricted RN license from the state in which he/she would be practicing.
