- Collecting and organizing client information and documentation.
- Listening to client concerns and ensuring their needs are addressed.
- Preparing and submitting intake packets for the HCBS program.
- Coordinating with care management professionals and external sources for required documentation.
- Maintaining accurate records and ensuring compliance with program requirements.
Ideal Qualifications:

- Previous experience in HCBS or care management intake is preferred but not required.
