Job DetailsJob LocationArizona Rehab Campus - Tucson, AZDescriptionJOB DESCRIPTION: The role of the Intake Coordinator is to provide evaluation and assessment services, assisting individuals in establishing and maintaining recovery from addiction while developing an initial treatment plan. The Intake Coordinator will aid individuals in improving their quality of life through initial contact and connecting them to social, spiritual, and peer supports. This position will also ensure the safety and well-being of clients while facilitating their integration into clinical programs under the supervision of the Case Management and Clinical Services teams.JOB REQUIREMENTS:Education: High school diploma or equivalent required. 2 years' experience in related field preferred; Bachelor's Degree Preferred.Experience: Prior intake, assessment, admission and crisis intervention experience with a substance use and mental health population, preferably in a residential setting.Certification: Healthcare Provider CPR, Level 1 Fingerprint Clearance Card and TB. Handle with Care within the first 90 days of hire and annually thereafter.KNOWLEDGE/SKILLS/EXPERIENCE:
  • Strong verbal and written communication skills in English
  • The ability to complete the clinical and risk assessment, possible in crisis situations, in a time effective manner ensuring all components are completed. This requires strong assessment skills and the ability to keep clients on track and focused on the assessment process.
  • Ability to work both as a team member and independently.
  • Understanding of relapse prevention and harm reduction techniques
  • Strong interpersonal skills
  • Crisis management skills
DUTIES/RESPONSIBILITIES:
  • Consistently provides supervision for assigned clients.
  • Clearly documents observations of client behavior.
  • Always ensures safety of client.
  • Obtains accurate vital signs, weight and height, and documents information appropriately.
  • Notifies Charge/Staff Nurse of clients with potential risk of injury due to falls, self-destructive, or assaultive behavior. Communicates the potential risk of client injury to others.
  • Demonstrates effective management of assaultive behavior and competence in Handle with Care.
  • Utilizes most facilitative measures to de-escalate clients, such as 1:1 conversation ( 1:1 ), diversional activities, groups, peer support, or decreased stimuli.
  • Completes intakes including but not limited to; completes vitals, UA drops, client belongings, inventory safe bag, contraband check, performs visual exam of the skin, and documents accurately.
  • Assist with restocking intake room and organizing property room.
  • Regularly assumes responsibility for client clothing and personal property by securing items in accordance with established procedures; accurately notes and provides safe care for client valuables.
  • Collaboratively completes service plans and ASAM.
  • Acts as a supportive member of the clinical team and assists clients in managing day-to-day issues.
  • Brings the concept of recovery and support to the clinical treatment team.
  • Assists the client in Intake process and coordination of care when clients arrive for admission.
  • Helps clients to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and client's part in creating them
  • Educates and supports clients in implementing a range of relapse prevention and harm-reduction strategies as AMA strategies
  • Works closely with the multidisciplinary team to ensure client needs are met
  • Actively participates in weekly supervision with Director of Case Management and/or the Clinical Director
  • Ensures Clients' Rights and Confidentiality are maintained.
