As a Navian Hawaii member, you will provide support for those who guide patients and their family on their journey, delivering comfort and support with a caring heart, replacing fear with hope, helping them find the beauty and joy in every moment, and providing each person with the kindness and dignity they deserve.Position Summary:The position under the direction of the Admissions Manager, supports all Admission RNs, ensuring excellent customer service, and that all referrals are handled in a sensitive and timely manner - demonstrating professionalism, speed and accuracy. The position is also responsible for tracking business data daily, weekly & monthly and reporting lo management, obtaining all necessary documents needed for admission, developing strong and meaningful relationships with referral sources, and ensuring communication and status updates with referral sources in a timely manner. The Intake Coordinator also keeps detailed records on all referrals, admissions and conversions for accuracy and tracking ii against monthly goals; reporting back to the Admissions Manager on a weekly basis.Primary Responsibilities:ln-Take:Responds to referrals immediately, assuring that all necessary information is obtained, communicated and acted upon appropriatelyEnsures that all patient information and required documentation is input into the electronic medical record system in a timely and accurate mannerMaintains knowledge of admissions criteria for hospice patients and demonstrates ability to incorporate applicable information into the referral/admissions processEstablishes clearly defined system for follow up of Pending Referrals and updates data base regularlyAssists admissions team in gathering needed paperwork and/or patient information, follow up activities and/or resolving problems (ie: insurance verification, CTI, Start of Care Orders, etc.)Builds relationships & communication with referral sourcesGoals / Reporting:Ensure accuracy and consistency of informationLog data into weekly reports at the end of each week and track against goalsShare weekly reports with the team Monday following the week priorReviews data and evaluates results in support of achieving goalsMakes necessary recommendations to ensure customer needs are met and in line with the overall strategic initiatives of the organizationReferral Sources:Strong customer service focusResponsible for establishing strong & meaningful relationshipsWork with referral sources and office staff to ensure transparent execution ofservicesEnsure status updates are made lo each referral source in a timely manner •Resolves customer complaints by investigating problems; deploying solutions;making recommendations to managementTeamwork:Practice effective communication and interpersonal skills to enhance positiveteamwork and accomplish goals/tasksEstablish working relationships with all company support departmentsGeneral:Represent the mission and value of Navian Hawaii with passion and enthusiasmPromotes excellence in customer relations in all encountersMust be able to plan and meet deadlinesMaintain a flexible work schedule to meet the demands of the jobConvey a professional and positive image and attitudeOther duties as assignedKnowledge and Skills Requirements:Strong communication and customer service skills and the ability to establish and maintain effective Working relationships with staff, patients/families, referral sources, physicians, community partners, and other agencies/organizationsExcellent organizational skills and the ability to prioritize, problem solve and manage multiple tasks simultaneouslyAbility to set priorities, research, problem solve and make independent decisions with minimal supervisionAbility to maintain highly sensitive and confidential materialAbility to initiate, develop and maintain relationships with referral sources and community partners including hospital discharge planners, physicians, vendors and nursing facility staffFamiliar with concepts and practices used in a hospice settingAbility to maintain records and prepare related reportsBasic computer knowledge of MS applications such as Word, Excel, Outlook,Working knowledge of departmental and organizational policies, procedures, and operationsExperience and Educational Requirements:High school diplomaMinimum one year experience in healthcare or related fieldKnowledge of hospital/physician referral process, end-of-life care preferredHours:This role works Tuesday through Saturday 8 am - 430 pmPosition may require after hour work including weekends and holidays, as neededEmployment Requirements:All offers are contingent on a candidate's successful completion of the following requirements:A legally compliant background checkDrug Screen & Physical ExamWhy Navian Hawaii?Consistent schedule Team-oriented, locally run nonprofit where every voice is heardCompetitive pay with an excellent benefits packageFlexible organization that supports positive work/life balance
