DescriptionWe are seeking a talented and dynamic Intake Referral Coordinator to join our team!The Intake Referral Coordinator will coordinate and facilitate admissions to Gentle Heart Recovery. Maintain communicationwith referents, patients and family members. DUTIES/RESPONSIBILITIES:
  • Answer and respond professionally and promptly to phone calls and e-mails.
  • Interviews prospective patients and collect referral information including referring agency or individual, patient demographics, medical, psychiatric, and substance abuse history and insurance.
  • Request/obtain necessary medical or psychiatric records when indicated and obtain clearances when indicated.
  • Able to verify insurance and utilize systems, including but not limited to, E-Pace, Medicaid eligibility site, etc.
  • Coordinate admit date, program, and transportation.
  • Accurately enter all data in the EMR system.
  • Follow up and confirm scheduled admissions.
  • Explain pre-certification process.
  • Inform patient/family regarding permissible and not permissible clothing and other items.
  • Maintain communication with marketing and referral sources. Notify both when scheduled admits do not occur as planned.
  • Utilize discharge and Census information when scheduling patients.
  • RequirementsREQUIRED SKILLS/ABILITIES:
    • Excellent interpersonal and communication skills, with the ability to interact compassionately with individuals battling substance issues.
    • Proficient in EMR, CRM software and Microsoft Office Suite.
    • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
    • Strong organizational and multitasking abilities.
    • Strong ability to manage multiple relationships with various agencies.
    • Ability to maintain confidentiality and act with discretion and empathy.
    EDUCATION AND EXPERIENCE:
    • Associate's Degree in a related field. BA or BS preferred.
    • Customer Service, Substance Abuse Treatment, and Intake Experience is preferred.
    • Two years' experience in field preferred.
