A DAY IN THE LIFE OF AN INTAKE COORDINATOR: This position, in coordination with discharge planners, is responsible for serving as liaison between VNAHSR and area providers/referral sources to ensure smooth patient transitions from hospital to VNAHSR care. In addition, this position is responsible for analyzing patient care needs from various referral sources and routing to appropriate VNAHSR programs. Responsibilities include but are not limited to:
- Reviews voicemail and takes appropriate action, including, but not limited to, new patient referrals, messages and up-dated medical requests, etc. Logs information and/or transfers information to appropriate clinician in a timely manner.
- Forwards physician's offices to Intake nurse for new orders.
- Reviews pending referrals for anticipated discharges from nursing homes. Provides clinical managers and schedulers with up-dated information in timely manner.
- Receives patient referrals from a variety of sources. Gathers preliminary information needed and may query referral source for additional information as appropriate. Actively communicates dates and referral source information to Clinical Managers and Schedulers to maximize effectiveness of referral.
- Forwards (up-dated) doctor's orders to Intake Clinician. Assists in the entering data EMR software in a timely manner and in accordance with agency policies and procedures. Takes action to ensure that orders are sent to appropriate case manager and supervisor.
- Actively participates in conference calls with referral sources (physicians, hospitals, members of the public, etc.) and provides information on VNAHSR services. Uses knowledge of VNAHSR services and community services to maximize opportunities to cultivate new customers.
- Contacts potential patients within 24 hours of referral in accordance with agency and CMS regulations to coordinate patient coverage.
- Records and maintains a variety of statistical data included but not limited to odd billing episodes, Not taken ups, referrals cancelled d/t other agency active.
- In collaboration with various VNAHSR clinical staff, takes action to confirm patient status (e.g. physical condition, discharge/admission to hospital, etc.) in order to maximize effectiveness of start/continuance of services.
Qualifications:
- High School diploma or equivalent required.
- One year of data entry or computer operator experience required. Knowledge of VNAHSR programs preferred. Knowledge of medical terminology required.
- Valid state driver's license required. Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required.
