Sacramento, CASCHEDULE Monday- Friday, 8:30 am to 5:00 pmAbout the Position…Intake Coordinator works under the Director of Behavioral Health Services' supervision to support data collection, program coordination, and scheduling. The Intake Coordinator will review and evaluate all applications to the program, determining eligibility and intake. This position is responsible for reviewing, evaluating, and verifying intake information and assuring compliance with program eligibility guidelines. The Intake Coordinator will have experience in providing care to low-income populations including those experiencing homelessness, a commitment to the provision of quality programs and services and the ability to work well with a diverse and multi-disciplinary team.You'll be responsible for…* Demonstrate proficiency in use of Microsoft Office Applications by maintaining spreadsheets, word documents and communication through email.* Facilitate Informational Orientation for prospective clients.* Complete intake assessments with the prospective clients.* Obtain and input all required client information into the Homeless Management Information System (HMIS) database: intake and exit data entry within 72 hours of client/family entry and exits, respectively; VI-SPDAT assessments, and any other assessments required by SSF.* Attended calls as needed with CAS providers and management.* Be a primary contact for referrals and coordinate communication and information sharing promptly.* Liaison with probation officers, law enforcement, substance use counselors, foster agencies, and other community groups for intake of potential program participants.* Process changes and updates to applicants' records to re-determine eligibility for programs as applicants' status changes.* Monitoring and updating the waiting list regularly.* Prepare regular summary reports of waitlist, bed count figures and program rosters.* Ongoing monitoring of client chart compliance and data entry related to intakes in internal and external databases and files.* Generate a variety of technical, statistical, and administrative reports to assist management in compiling information necessary to comply with county, state and federal requirements and audits.* Participate in program, department and agency meetings and training as required, as well as inter-agency groups that provide assistance to individuals experiencing homelessness.* Maintain and execute confidential information according to HIPAA standards.* Attend staff training courses and department meetings as needed and other duties as assigned.* Perform administrative tasks such as email, phone messages, and compile daily reports and monthly statistics.* Maintain excellent verbal and written skills, including clear and effective communication with others (staff, public, volunteers) that is professional, respectful, positive, and educationalYou must have...The ability to communicate and comprehend oral and written information, analyze information, make reasonable decisions. Time management, organizational, and problem-solving skills are necessary.While performing this job, this position typically spends long periods of time sitting, standing, climbing stairs, walking and carrying and lifting 20 (lbs). It requires coordination of multiple tasks, performing repetitive motion activities (typing), and firm/strong grasping. Operation office equipment such as phone, fax, copier, and computer. Reasonable accommodations may be made to enable individuals with disabilities to perform essential job functions.Education/Experience/Training:* BA Degree or four years of experience in social services or hospitality, including experience dealing with substance use in clients and crisis intervention, reentry programs, and mental health.* Proficiency in Microsoft Office and Web-based programs, HMIS and any other database software(s) used to track work outputs.* CPR/First Aid Training Certification and HMIS Training Certification within 3 months of hire.* TB Test Results required before hire.* Valid CA Driver's License, auto insurance and reliable transportation.* Type at a speed of at least 45 words per minute.Physical RequirementsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
