Our wide range of services includes school-based and outpatient therapy, psychiatric evaluations, community support, and substance abuse treatment.Position OverviewWe are seeking a full-time Intake Coordinator to provide community-based and in-home individual and family therapy, as well as community support services. The Intake Coordinator will work closely with clients, families, and their natural supports to assess strengths and needs, coordinate service implementation, and ensure quality and compliance in client documentation. This position plays a key role in facilitating access to care and supporting the ongoing needs of individuals and families.Location: DurhamCompensation: starting at 54,373Key ResponsibilitiesTherapeutic Services
- Deliver in-home and community-based therapy for children, adolescents, adults, and families.
- Assess strengths and needs of clients, families, and natural supports to develop appropriate treatment plans.
- Recommend and implement evidence-based treatment modalities, providing services directly or through referrals.
Intake and Service Coordination
- Conduct initial intake assessments with families and/or community agencies requesting services.
- Assist with client enrollment and authorization for services through LME/MCOs.
- Facilitate appropriate service matches based on individual client needs.
Documentation and Compliance
- Develop and maintain Person-Centered Plans (PCPs), NC-TOPPS assessments, and Diagnostic Assessments.
- Ensure compliance with quality assurance standards for all client documentation.
- Communicate with LME/MCOs regarding service authorizations and track reauthorization requirements.
Collaboration and Advocacy
- Work collaboratively with staff and management to address client staffing needs.
- Provide training to employees on authorization processes and documentation requirements.
- Act as a community advocate, connecting clients to resources and providing ongoing support.
On-Call Support
- Participate in a 24-hour rotating on-call schedule to respond to client and staff needs as necessary.
Qualifications
- Education: Master's degree in Counseling, Psychology, Social Work, or a related field.
- Licensure: Must hold an associate or full Mental Health license (e.g., LCSW/A, LCMHC/A, LMFT/A).
- Skills:
- Strong organizational and communication skills.
- Ability to collaborate effectively with multidisciplinary teams.
- Proficiency in maintaining accurate and timely documentation.
